A Case of Diffuse Pericardial Mesothelioma with Intracardiac Extension

Abstract

A case of diffuse mesothelioma of pericardium with unusual intracardiac extension was reported. The patient was 49 year old male admitted due to dyspnea for 40 days. Physical examination and non-invasive cardiac evaluation revealed right-sided heart failure, pericardial effusion and left atrial masses. Pericardial tube drainage was performed but specific diagnosis was not made.
Pre-operative diagnosis was left atrial myxoma and pericardial effusion of unknown cause. Operative findings were friable masses in pericardial cavity around superior vena cava, transverse sinus and left atrial posterosuperior wall. Two right atrial and a left atrial masses were seen, being connected to the pericardial masses.
Histologic diagnosis of the masses was diffuse mesothelioma. The patient died of pneumonia at 91st post-operative day despite of therapeutic trials of radiation and chemotherapy.
Clinical differential diagnosis, therapeutic and pathologic implications were discussed.
